What is the difference between a protocol deviation and a violation at your site?

Most sponsors use 'deviation' for anything that departed from the protocol and 'important deviation' or 'violation' for the ones that affect safety or data integrity. Your protocol or monitoring plan should define it; if it does not, ask before you categorise anything.
